Sign Up

To sign up to be a provider on one of EMI Health's participating provider panels, please follow these steps:

• Call EMI Health providers relations department at 800-662-5850.
• Let the providers relations representative know that you would like to sign up to be a health care provider on the EMI Health Network.
• Provide the providers relations representative with the provider's name, degree, specialty type (if applicable), address, and telephone number.
