Premier (High)

The Premier High dental plan allows you to see any provider you chose; however, when you receive care from one of our 1500+ in-network providers, you will receive 100-percent coverage on preventive care with no waiting periods. The plan includes 50-percent coverage on orthodontic care for youth and has the lowest deductible.

Premier (Low)

The Premier Low dental plan is similar to the Premier High, only with a slightly higher deductible. The plan includes a 25-percent discount on orthodontic coverage for all covered dependents.

Advantage

The Advantage plan gives you peace of mind knowing what your out-of-pocket expense will be before going to any of our 1000+ in-network providers. The plan uses a set copay fee schedule for services and has 100-percent coverage on in-network preventive dental care. Included is a 25-percent discount on in-network orthodontic care.

Value

The Value plan is a discount dental plan that allows participants to receive up to 70-percent savings on preventive care, 60 percent on basic care, and 50 percent on major dental services with no waiting periods. The Value discount plan is in-network only and has the lowest monthly fee. Note: This is not an insured product.
