type of Term Insurance is it?
Age related and premiums are on the
application.

How
are the premiums paid?
Payroll deduction.

Example: For
a non-smoking 30-year-old person, the premium for $50,000 is $3.00 per month.

Rates are on the
second page of the application.

How
much coverage can I have?
Face value has an upper limit of
$500,000 or five times your salary and must be in increments of $10,000.
·
Spouse’s limit is
the lesser of $200,000 or 50% of the employee’s amount.
·
Guaranteed issue
for employee is $100,000 (Guaranteed issue means there is no requirement for a
physical).
·
Guaranteed issue
for spouse is the lesser of $50,000 or 50% of insured employee.

do I enroll?
Complete the enrollment application
and send to Johnson Rooney Welch, Inc., 2250 Douglas Blvd. Suite 210, Roseville,

When
can I enroll?
Guaranteed issue must be submitted
within 30 days of your hire date.
If
it is not a guaranteed issue then enrollment can be any time during the year
and a physical may be required.
